This policy explains how US Health Insurance Options LLC handles information submitted through this website. Insurance applications, insurer systems, and other services may have separate privacy notices.
Information you provide
When you request a coverage review, the form collects your name, email address, phone number, state, ZIP code, selected coverage, date of birth, gender preference, coverage timing, and preferred contact method. Depending on your answers, it may also collect spouse or dependent information, business and employee-count information, current coverage, work situation, and an estimated income and tax household size if you choose to explore income-based savings.
You may choose to discuss income privately. Please do not enter Social Security numbers, detailed medical information, medical records, payment-card information, passwords, or insurance-portal credentials in this form. Provide information about a spouse or dependent only when you have authority to do so.
How the information is used
The information helps Rachid prepare for your coverage review, respond to your request, and contact you by your selected method. Submitting the form does not enroll you in insurance or authorize access to a Marketplace account.
The form also records your consent choices and the version of the consent notice. If your visit includes referral tags, it records those tags and a referring website address to help identify where the request came from.
Storage and service providers
The website is hosted through OpenAI Sites. A submitted request passes through the website’s server and a Google Apps Script to a restricted Google Sheet used by US Health Insurance Options LLC. An internal email alerts the advisor to a saved request and links to the restricted sheet; the notification does not include the submitted birth dates or income.
OpenAI Sites and its hosting infrastructure, Google Sheets, Google Apps Script, and Google’s mail services process information needed to operate this workflow. Hosting providers may also process technical information such as IP addresses, browser information, security cookies, request logs, and traffic analytics. Their own privacy terms govern their services.
The current website intake does not automatically enroll you with an insurer or distribute your submission to a lead marketplace. Any separate application or insurer submission requires its own process and applicable authorizations.
Contact and optional email updates
The required contact checkbox permits follow-up about your coverage request by phone or email. Optional health-coverage email updates use a separate checkbox. You can withdraw permission for optional updates by using the unsubscribe instructions in an update or by contacting us. You can also tell us that you no longer want follow-up about your request. The current website workflow sends an internal lead alert, not an automatic marketing campaign.
Appointment booking and external links
If you use an appointment calendar linked from this website, the calendar provider collects the information needed to arrange and confirm your appointment. Google Calendar may be used for scheduling; an existing AgentCRM calendar may be linked while that connection is being set up. Review the provider’s privacy information before submitting information on an external page.
Links to social networks and other websites take you to services with their own privacy practices. We do not control those websites.
Retention, security, and your choices
Submitted information is retained for handling your request, providing related service, and maintaining records where required. The website currently does not apply an automatic deletion deadline to submissions. You may request access, correction, deletion, or withdrawal of contact permission using the contact details below. We may need to verify your identity or authority, and some records may need to be retained to meet legal or compliance obligations.
Access to the lead sheet is restricted through Google account permissions, and the connection token is kept in server-side settings. No online system can guarantee complete security. Do not use the website form to transmit the sensitive information listed above.
Requests involving children
This website is intended for adults seeking coverage guidance, including parents or authorized guardians seeking coverage for dependents. Children should not submit the form themselves. If a child has submitted information directly, contact us so the request can be reviewed.
Changes to this policy
Changes will be posted on this page with an updated date. A change to this policy does not by itself give us permission to use information for an unrelated purpose.
