These terms describe use of the US Health Insurance Options LLC website and its coverage-review and appointment-request features.
Purpose of the website
The website provides general health-insurance education and a way to request personal guidance from Rachid Eloudiyi. General content does not replace the terms of an insurance policy, an insurer’s current plan documents, or advice specific to your circumstances. Any wellness information is general education and does not replace medical care.
A request is not an application or enrollment
Submitting the website form or booking an appointment does not apply for insurance, enroll you in a plan, bind coverage, determine your eligibility, or authorize an advisor to access or submit a Marketplace application. Applications and enrollments require a separate process, your review, and applicable authorizations.
Plan availability, rates, benefits, eligibility, provider participation, effective dates, and application requirements depend on the plan, location, and circumstances. Coverage is subject to the applicable policy and enrollment requirements. Verify important details with the insurer and provider before relying on them.
Information you submit
Provide accurate contact and coverage information and only submit another person’s information when you have authority to do so. Do not submit Social Security numbers, detailed medical information, medical records, payment-card information, passwords, or insurance-account credentials through this form.
Do not use the website for false requests, spam, impersonation, unlawful content, attempts to access other people’s information, or interference with the website’s operation.
Consultations and appointments
The initial assessment offered on this website is a free 15-minute consultation. An appointment request is confirmed through the scheduling system. If you cannot attend, use the confirmation’s cancellation or rescheduling option or contact Rachid.
If a separate paid professional service is offered, its scope and price must be disclosed and agreed separately before that paid work proceeds. This website form does not authorize a charge.
Communication and privacy
The form’s required contact permission covers follow-up about the submitted request by phone or email. Optional email updates require the separate opt-in choice. You can contact us to withdraw permission for further follow-up or optional updates.
How this website handles submitted information is described in the Privacy Policy. Any insurer application, Marketplace authorization, or other service may involve separate notices and agreements.
External services and content
Appointment scheduling, social-network links, insurer resources, and other external services are governed by their providers’ terms and availability. The website may link to Google Calendar or the existing AgentCRM appointment calendar. An external link does not mean that US Health Insurance Options LLC controls that service.
Blog articles carry their original publication dates. Enrollment rules, tax provisions, product availability, and plan terms can change. Confirm the rules for your state and coverage year before acting on dated content.

Website content and features may be updated or temporarily unavailable. If a form submission does not receive a success confirmation, contact Rachid rather than assuming that a request was saved. An urgent medical need should be addressed through an appropriate healthcare or emergency service.
